Registration Form

I hereby declare that I have read, understood, and agreed to the Terms and Conditions of the Asian Football Academy (AFA), and confirm the registration of my child accordingly. This includes agreement with the following documents:

• Academy Rules and Code of Conduct
• Media & Image Rights Consent
• Data Privacy and Protection Policy

I certify that my child is physically and mentally fit to participate in sports activities and has no medical conditions that would limit or endanger their involvement in training sessions, matches, or any physical activities organized by the Academy.

I acknowledge that my child will participate in various football-related activities, including but not limited to training sessions, matches, fitness drills, and off-site events, and I consent to their full involvement in such programs.

I accept and assume all risks related to physical activity, including potential injuries, accidents, or health incidents that may occur during participation, whether on or off Academy premises.

I understand that while AFA takes all necessary safety precautions, the Academy, its coaches, staff, management, affiliates, and facility partners are not liable for any injuries, health complications, or damages—whether minor or serious—that may arise as a result of participation in sports activities.

I confirm that all medical and personal information provided is accurate and up to date, and acknowledge that withholding relevant information may affect the safety and well-being of my child during Academy programs.

I agree to comply with any updates to the Academy’s Terms and Conditions, as may be amended from time to time, and understand that failure to adhere to the rules may result in suspension or termination of participation.

By signing this declaration, I voluntarily release and discharge the Asian Football Academy W.L.L. and its associated personnel from all claims or liabilities arising out of my child’s involvement in Academy activities.
