I thus declare that I have read, understand, agree to be bound by, and have registered my kid for the Asian Football Academy in accordance with the following terms and conditions:

Image Right & Media Consent
Data Protection Policy
Covid-19 Declaration

I certify that my child is fit enough to engage in sports, healthy, and free from any physical or mental conditions that might prevent them from doing so.

I agree that my child participates in all the activities of the Asian Football Academy and uses all its sports equipment and facilities, and

Accept and assume all risks resulting from the participation of my child to the Asian Football Academy activities in any locations.

I have truthfully and completely stated everything that could have a potential impact on the Academy's decision to approve my child's enrollment.

I declare that I have received, read, fully comprehended, and agreed to the Academy Terms and Conditions, as they may be revised from time to time and into which I thus agree.

I hereby expressly and explicitly release the Asian Football Academy W.L.L. and/or its coaches, personnel, officers, directors, affiliates, agents, contractors, and the venue from any liability and will not hold them responsible for any injuries, losses, liabilities, damages, losses, damages caused by my child, injuries, permanent disability, or death that my child may suffer or incur;

while attending and/or taking part in any Asian Football Academy events.
